    Patient Portal

    If you are a current patient or returning patient and have not yet gotten an invitation to access the portal, please email [email protected] and request access.

    Please note all our scheduling is done through AppointmentQuest.com.  You cannot schedule or cancel an appointment in the portal (though it may say different). 


    We are currently using a secure patient portal for all of our forms.  Please look for an email from “[email protected] for access to the secure patient portal.

    Once you schedule a new patient appointment you will receive access to the secure patient portal where all the forms can be found and completed online. Please look for an email from [email protected] which will provide information on how to access the portal. This will usually come within 24 hours of scheduling your appointment.

    Should you have difficulty accessing the portal or prefer to complete the forms by hand, they are all provided below. Once completed we prefer for you to upload these forms to your portal. Please do not take individual pictures of the forms, but scan them and upload each form in its entirety labeled. The free App Scanner Pro can do this easily from your phone. All forms can be completed directly online easily. 

    If you have difficulty uploading them you may email the forms to [email protected] or fax to 858-400-5204 at least 24 hours prior to your appointment..

    All New Patients

    New Patients Must Complete Forms prior to their appointment. All forms can be completed directly online.  

    If you prefer, you may download the forms here.  New patients must complete forms (1-6) below, as well as the appropriate history form, and upload them to you secure portal (or email/ fax) prior to your appointment. Once you confirm your appointment you will receive an email allowing access to your portal. All of these forms will be there.
